Incoming Students

Welcome to Simmons!

All graduate and undergraduate students enrolled for 9 or more credits must demonstrate that they have health insurance. See the Health Insurance section for more information.

Health requirements for undergraduate and graduate students are detailed below. If you have any questions about these health requirements or using the Student Health Portal, please call the Health Center at 617-521-1020. We appreciate your cooperation.

Undergraduate Students

The Commonwealth of Massachusetts requires by law that all full time (9 credits or more) students submit documentation of immunizations before starting classes. To comply with the law, you must submit your immunization records to Simmons Health Center by July 15 for fall classes and January 10 for spring classes. Please note that it is necessary to have met all immunization requirements in order to register for spring classes.

All newly-enrolled full time students 21 years of age and under need a meningitis vaccine or signed waiver. Signing the waiver as allowed by state regulation does not prevent you from receiving the vaccine at a later date, and in fact, we recommend that you do so.  New undergraduate students are required to complete and upload the form forms included in the Entrance Health Packet:

  • Entrance Information
  • Student Health History
  • Tuberculosis Risk Assessment
  • Immunization Form (or provide a complete and legible copy of immunizations) 

The Entrance Health Packet and other forms are available on the Student Health Portal. In addition to completing the forms, immunizations must be entered directly into the Immunization record. 

Please be advised that some incoming students are not yet able to log in to the Portal. If you are unable to access the Portal, the Entrance Health Packet can be downloaded here. We recommend that you keep a copy of your completed forms for your records. For assistance obtaining and submitting the required documentation, or if you have any other questions, please call Health Services at (617) 521-1020.

Graduate Students
