Immunizations & New Student Requirements

All graduate and undergraduate students enrolled for nine or more credits must demonstrate that they have health insurance. See the Health Insurance While at School

Undergraduate Students

Undergraduate requirements for students not in a Health Science program

The Commonwealth of Massachusetts requires by law that all full time (9 credits or more) undergraduate students submit documentation of immunizations before starting classes. The state immunization requirements are outlined below. To comply with the law, you must submit your immunization records to Simmons Health Center by July 15 for fall classes and January 10 for spring classes.

VACCINE REQUIREMENTS

  • 3 dose series of hepatitis B vaccine OR blood tests that shows proof of immunity
  • 2 doses of MMR vaccine (measles, mumps, and rubella) OR blood tests that shows proof of immunity
  • 2 doses of varicella vaccine OR blood tests that shows proof of immunity OR a history of chickenpox disease documented by a health care provider
  • 1 dose of Tdap vaccine after age 11 (tetanus, diphtheria, pertussis) then a tetanus booster (Td) every 10 years
  • Meningitis vaccine or signed waiver
  • Tuberculosis risk assessment form

Residential students need a Meningitis vaccine or signed waiver. Please note that it is necessary to have either the vaccine or a signed waiver in order to receive your room key upon arrival. Signing the waiver as allowed by state regulation does not prevent you from receiving the vaccine at a later date, and in fact, we recommend that you do so.

The Commonwealth of Massachusetts requires by law that all full time (9 credits or more) undergraduate students submit documentation of immunizations before starting classes. The state immunization requirements are outlined below. To comply with the law, you must submit your immunization records to Simmons Health Center by July 15 for fall classes and January 10 for spring classes.

VACCINE REQUIREMENTS

  • 3 dose series of  hepatitis B vaccine AND blood test that shows proof of immunity

  • Rubella titer (blood test showing immunity) in addition to (2) MMRs
  • 1 dose of Tdap vaccine (tetanus, diphtheria, pertussis) then a tetanus booster (Td) every 10 years
  • 2 varicella vaccines or a titer that show proof of immunity. A history of the disease is not acceptable as proof of immunity for clinical participation.

Please print and complete the applicable Entrance Health Certificate below. These pages cannot be completed electronically. All forms must be returned to the Health Center by fax or mail by the due date. If you have any questions please call the Health Center at (617) 521-1020.
We recommend that you keep a copy of your completed forms for your records.
