Immunizations & New Student Requirements

These web pages will outline the immunization records and other health information required of entering undergraduate and graduate students. Please follow the instructions in the appropriate section below:

  • Undergraduate students including Dix Scholars, transfer students and students in the undergraduate Health Sciences programs
  • Graduate students not in the Health Sciences
  • Graduate students in the Health Sciences

Note also that all graduate and undergraduate students enrolled for nine or more credits must demonstrate that they have health insurance. See the Health Insurance While at School section of this web site for more information.

Undergraduate Students

Undergraduate Requirements

The following immunizations are required by Massachusetts law for all full time entering undergraduate students including transfer students, and both full and part time undergraduate Health Science students

  • 2 MMR (measles, mumps, rubella) vaccines, the first given no earlier than one year after birth and the second at least 1 month after the first, or serology tests (titers) for measles, mumps and rubella that demonstrate immunity
  • 1 Tdap (tetanus, diptheria, pertussis) booster within the last ten years.
  • Three hepatitis B vaccines or a hepatitis B serology test (titer) that demonstrates immunity.
  • Two varicella (chicken pox) vaccines or a history of varicella verified by your health care provider, or a varicella titer that demonstrates immunity
  • A meningitis vaccine or a signed meningitis waiver if you plan to live on campus. Please note that it is necessary to have either the vaccine or a signed waiver in order to receive your room key upon arrival. Signing the waiver as allowed by state regulation does not prevent you from receiving the vaccine at a later date, and in fact, we recommend that you do so.

The Massachusetts Department of Public Health strongly suggests TB testing (PPD) for students who are at increased risk for exposure to tuberculosis. Please download and review the TB Risk Questionnaire below. If you have answered "yes" to any of the questions please bring the form to your health care provider for review.

Additional requirements for full and part time undergraduate health science students

Although not required by law, health science students who will have clinical placements will need to have:

  • A rubella titer demonstrating immunity
  • A varicella titer demonstrating immunity or 2 varicella vaccines (a history of varicella will not be accepted)

Instructions for Filing Undergraduate Records

In order to have your complete health record on file at school, we ask that you download and submit the 5 page Health Certificate below which will put you in compliance with school policy and state law. Completed pages must be returned to the Health Center by fax or mail before your arrival at school. Please note that these pages cannot be completed electronically. If you do not have access to a computer to download the form please call us at 617-521-1020.

We recommend that you keep a copy of your completed forms for your records.

Graduate Students

Instructions for Graduate Students not in Health Science Programs

The Commonwealth of Massachusetts requires by law that all full time (9 credits or more) graduate students submit documentation of several immunizations before starting classes. The State immunization requirements are outlined for you below. To comply with the law, you must submit your immunization records to Simmons Health Center by August 1 for fall classes, Jan 10 for spring classes, and May 1 for summer classes.

Vaccine Requirements

  • 3 dose series of hepatitis B vaccine OR blood tests that shows proof of immunity
  • 2 doses of MMR vaccine (measles, mumps, and rubella) OR blood tests that shows proof of immunity
  • 2 doses of varicella vaccine OR blood tests that shows proof of immunity OR a history of chickenpox disease documented by a health care provider
  • 1 dose of Tdap vaccine after age 11(tetanus, diphtheria, pertussis) then a tetanus booster every 10 years

Please fill out the Graduate Immunization Form or obtain a copy of your immunization records and submit these to the Simmons College Health Center by mail, fax or email to healthcenter@simmons.edu.

Steps to help you obtain your immunization records:

  • Contact your current health care provider
  • Contact your childhood health care provider
  • Contact your previous schools (college(s), high school, or primary school) that may have required your immunization history

If you are still unable to obtain your immunization records or if you actually have had the illness:

  • Ask your health care provider to order blood tests that proves immunity and submit the results to the Simmons College Health Center.
    OR
  • Obtain the required vaccinations from your health care provider. Many local pharmacies also administer these vaccines; they may be covered by your health insurance.

Instructions for Full and Part Time Graduate Students in the Health Sciences

The Commonwealth of Massachusetts requires by law that all full & part time graduate students in a health science program submit documentation of several immunizations before starting classes. The State immunization requirements are outlined for you below. To comply with the law, you must submit your immunization records to Simmons by August 1 for fall classes, Jan 10 for spring classes, and May 1 for summer classes.

Vaccine Requirements for All Health Science Students

  • 3 dose series of hepatitis B vaccine OR blood work that shows proof of immunity
  • 2 doses of MMR vaccine (measles, mumps, and rubella) OR blood work that shows proof of immunity
  • Rubella titer (blood work) showing immunity (this is in addition to 2 MMRs)
  • 2 doses of varicella vaccine OR blood work that shows proof of immunity (History of the disease does not meet the requirement for health science students)
  • 1 dose of Tdap vaccine after age 11 (tetanus, diphtheria, pertussis then a tetanus booster every 10 years)

Please fill out the Graduate Immunization Form or obtain a copy of your immunization records and submit these to the Simmons College Health Center by mail, fax or email to healthcenter@simmons.edu.

Here are some steps to take if you are having difficulty in obtaining your immunizations:

  • Contact your current health care provider
  • Contact your childhood health care provider
  • Contact previous school(s) that may have required your immunization history

If you are unable to obtain your immunization records, contact your health care provider to obtain the necessary blood tests or vaccinations. Many local pharmacies also administer immunizations, which may be covered by your health insurance.
