Recovery Team Leader (CBFS)

Job Description
The Recovery Team Leader is responsible for ensuring the coordination of rehabilitative services for an assigned caseload of CBFS people and integration/coordination of all clinical and rehabilitative services for this caseload. This position provides team leadership, supervision of a team of Recovery Support Specialists, Peer Specialists and Employment/Educational Specialists who provide a varied array of services to persons in the community. The Recovery Team Leader is also responsible for the assignment and deployment of other Recovery Specialists (substance use, health and wellness, etc.) in accordance with each person's Individualized Action Plan. The Recovery Team Leader also works closely with other NSMHA staff, other service providers, guardians, family members, community contacts, and Department of Mental Health staff.
Requirements/Qualifications
Master's Degree in Social Work or Psychology or related field preferred. Licensed Independent Clinical Social Worker (LICSW) or Licensed Mental Health Counselor (LMHC) preferred. Previous experience in the provision of clinical or rehabilitative services to adults with chronic and persistent mental health issues preferred. Previous supervisory experience preferred, but not required. Must have driver's license. Must be MAP certified and CPR/1st aide certified within the first three months of hire.

Manages and evaluates the design, development, and coordination of sexual assault and dating violence education programs with emphasis on freshmen education efforts as well as upper class and student leader-targeted programs. Implements general interest educational opportunities for the College. Designs, produces and creates educational and outreach materials on sexual assault and dating violence prevention for the College. Develops and administers office website. Performs a broad range of administrative support responsibilities including overseeing leases and maintenance of office equipment, processing student payroll, and ordering office supplies. Participates in university and community committees and task forces as needed. Acts as liaison with the community, students, and staff in facilitating university programs. Assists the Director in developing objectives and activities based on departmental goals. Prepares administrative reports documenting outreach and prevention efforts; analyzes data for predicting resource needs and assists the Director in developing long range education and outreach plans; assists with the development of long- and short-range office goals. Provides individual support and assistance as needed for students who have experienced sexual assault or relationship violence. Occasional on-call coverage of after-hours crisis response pager system. Other duties as required. This position reports to the Director of the Office of Sexual Assault Prevention and Response.
Requirements/Qualifications
BA/BS 1-3 years experience in sexual assault education, risk and prevention strategies, health education, counseling or related field.
Master's Degree preferred; the successful candidate will have experience developing and implementing violence prevention and education programs targeting young adults. Excellent written and oral communication skills. Ability to handle sensitive and confidential matters with discretion, and ability to work effectively with student, faculty and administrative constituencies required.

Job Description
The Social Worker will abide by and practice according to company Social Service Policies and Procedures Manual, State and Federal guidelines and the National Association of Social Workers' "Code of Ethics". Social Service functions include but are not limited to: supervising social service staff, direct services, advocacy, counseling, service coordination, education, in-servicing, policy and program development and community relations.

Essential Duties and Responsibilities:
- Assist with planning and implementing a comprehensive social service program which provides for counseling and other support services for residents and families in the Center.
- Identify medical related social needs of residents, provides appropriate services to meet the individual, as well as collective needs of residents and maintains records relating to the residents' social work needs and care.
- Consult with the Director of Social Services and other department heads regarding interdisciplinary issues, as well as maintenance of records.
- Work directly with residents and families experiencing personal and environmental difficulties or concerns related to the resident's physical or emotional condition.
- Promote the preservation of the resident's physical and mental health and to prevent the occurrence or progression of personal or social problems.
- Maintain written record of the frequency and nature of the social service consultation and services provided.
- Evaluate each resident's social needs then creates a plan for providing care and records the plan in the resident's medical record and periodically re-evaluates in conjunction with the resident's total plan of care.
- Plan and implement family meetings, upon admission and thereafter, to provide a forum for ongoing discussions between resident/family and interdisciplinary team.
- Perform initial assessments of each resident's psychosocial needs to be placed on the medical record.
- Written documentation at least quarterly is required for each resident. Short term residents require ongoing written documentation.
- Develop social service component of the resident's plan of care, identifying specific problems, goals and approaches.
- Participate and coordinate interdisciplinary care planning conference at the time of admission and at appropriate intervals thereafter in order to assist develop and revise the overall plan of care for residents.
- Ensure and document that residents and/or families have input into the care planning process.
- Act as a liaison between residents, families, outside agencies and the Center Administrator to ensure that the resident's rights are maintained and upheld.
- Document and prepare any reports requested by Administration, also counsels and provides assistance when change in financial status occurs for residents in the Center.
- Coordinate discharge planning and assists with developing an organized discharge plan for all residents.
- Concern self with the safety of all Center residents in order to minimize the potential for fire and accidents.
- Ensure that the Center adheres to legal, safety, health, fire and sanitation codes by being familiar with his/her role in carrying out the Center's fire, safety and disaster plans and by being familiar with current MSDS.
- Ensure that residents and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals' needs and rights.
- Perform other duties as requested.
Requirements/Qualifications
Education:
- Master's Degree of Social Work preferred or a Bachelor's Degree in Social Work.
- LCSW/LICSW preferred.
Professional Experience:
- Long-term care experience preferred.
- Mental health background desired.

BPHC has launched the *Racial Justice and Health Equity Initiative*, an organizational transformation process which aims to integrate health equity and racial justice principles and practices into all Commission work, both internal and external, in order to measurably reduce inequities in Boston. The Initiative's *Professional Development Series* includes a 22-hour learning experience for BPHC's 1,100 staff members. The Series focuses on racism, social determinants of health, health inequities, and strategies to advance health equity in all of the Commission's work.
The *Senior Trainer* will be the primary facilitator for the Series, conducting sessions along with a cadre of trained staff facilitators from across the organization. The Senior Trainer will co-facilitate approximately three two-day sessions per month, half-day practice workshops, organize professional development and support for staff facilitators, contribute to ongoing curriculum development, and become BPHC's internal expert on racial justice/racism and health equity theory and practice-based strategies.
