The Boston Semester application and subsequent forms are available to download as pdf files. Please note that you will need the Adobe Acrobat Reader plug-in to view and print these files.

Please submit the following materials:

1. Completed and signed Boston Semester at Simmons application form (49k pdf)
   
2. Completed form for Course Approval- Registration -Transcript Request (30k pdf). To be sure that your home institution will accept the courses you select for transfer credit, you must complete this form and have it signed by the academic dean of your home institution.
   
3. Official transcripts from all previously attended institutions. Official transcripts must bear the college stamp and arrive in sealed envelopes. Contact the Registrar at each college you have attended and ask that an official transcript be sent directly to:
