To apply to the Simmons College Upward Bound Math/ Science Program, you must submit the following information:
(All application materials are confidential and for program use only.)

1. A completed and signed application form. (An incomplete application will be returned to you for completion and may cause a delay in the admissions process).
   
2. Income verification.
   a.) A copy of your parent(s) most recent 1040
   Federal Income Tax Form
   or
   b.) A letter from the Welfare Office or Social Worker stating the family's financial situation
   
3. A copy of your school transcript of grades.
   
4. Verification of citizenship
   a.) copy of your birth certificate.
   or
   b.) A copy of your alien registration card.
   
5. Student Evaluation Form from both your current math and science teachers.

After you submit all of the information requested above, your eligibility for the Program will be determined and you may be contacted for an interview. If you have any questions, please contact us at (617) 521-2620.
